Young people with high rather than low exposure to alcohol marketing had a 50% higher relative risk of subsequent drinking outcomes in a systematic review covering 82,498 participants. The result describes an association across observational studies.
The review and meta-analysis, published online by JAMA Pediatrics on August 24, 2026, combined evidence from 23 publications representing 19 independent cohorts in 10 high-income countries.
Researchers examined longitudinal studies, which measure exposure and then follow participants over time. The eligible populations were younger than 25, and the outcomes included starting to drink and binge drinking.
That design provides stronger evidence about timing than a survey taken at one moment.
What the 50% figure means
The pooled relative risk for high compared with low marketing exposure was 1.50. The 95% confidence interval ranged from 1.37 to 1.63, meaning the estimated association remained above no difference across that statistical range.
Relative risk compares the probability of an outcome between two groups. It is not the same as a percentage-point increase.
For example, if 20% of a low-exposure group experienced an outcome, a relative risk of 1.50 would correspond to 30% in a comparable high-exposure group. That would be a 50% relative increase but a 10-percentage-point absolute increase. This is only an illustration because the underlying drinking rates and definitions differed among the studies.
The high-to-low comparison also combined several ways of measuring exposure. It should not be read as one universal advertising threshold that applies to every country, platform or age group.
The evidence covered several forms of marketing
The review included television and online advertising, alcohol portrayals in entertainment, promotional activities, branded merchandise, liking or recognizing advertisements and brands, and measures that combined several kinds of exposure.
Across 94 relative-risk estimates, 86% pointed toward a positive association between marketing exposure and later drinking. Television advertising produced the strongest association in the subgroup analysis.
That does not establish that television is necessarily the most influential channel today. The included cohorts were conducted over different periods, while digital marketing continues to change how brands reach consumers. The researchers called for future studies to use measures that capture several channels together.
The World Health Organization has made a similar measurement and policy point. Digital advertising can be targeted using online activity and personal data, making restrictions and exposure monitoring more difficult than with a conventional broadcast advertisement.
Longitudinal evidence still has limits
Following participants over time helps establish that the recorded marketing exposure came before the later drinking outcome. Even so, the studies were observational rather than randomized experiments.
Family circumstances, peer drinking, personality, income and existing attitudes toward alcohol could affect both exposure to marketing and subsequent behavior. Researchers can adjust for measured differences, but they cannot guarantee that every relevant factor has been captured.
All the included studies were judged to have some risk of bias because drinking outcomes were self-reported. Exposure was also measured in different ways, from participants’ estimates of how much advertising they saw to recognition of advertisements or ownership of branded items.
The authors rated the evidence as moderately certain. They upgraded the assessment because the studies showed a dose-response pattern, with greater exposure generally associated with greater drinking risk. Moderate certainty is stronger than a tentative signal, but it is not a claim that causation has been proved beyond doubt.
Why the study matters to the marketing business
The analysis did not evaluate one alcohol company, campaign or advertising restriction. It also did not measure sales generated by marketing. Its conclusion is narrower: across a large body of longitudinal research, higher exposure was associated with a greater risk of later youth drinking outcomes.
For alcohol producers, advertising agencies, digital platforms, sports organizations and entertainment companies, the result adds to the evidence regulators may consider when setting rules for the placement, targeting and monitoring of alcohol promotion.
It also complicates the familiar distinction between advertising intended to win market share and advertising that may expand consumption. A campaign can be designed for adults while still reaching younger audiences through shared television, entertainment, sports and digital channels.
The research team said more confirmatory work is needed in low- and middle-income countries, where alcohol marketing is expanding. Better measurement will also be necessary as exposure becomes distributed across more platforms and is less easily observed by researchers, parents and regulators.
